Ozark Center is committed to helping your child succeed. We provide a variety of mental and behavioral health services for children and adolescents. Our caring specialists use a family-centered approach and tailor your child's comprehensive treatment plan to his or her needs.


Getting Ready for Your First Appointment

We’re excited to meet you and your child! To make sure everything goes smoothly, there are a few important documents we need you to bring. These help us confirm who can make decisions for your child and, if needed, determine eligibility for financial assistance.

What You’ll Need to Bring

1. Proof of Custody (Required for All Clients)
We must have documentation showing who has legal custody of your child:

  • Birth Certificate – If both parents are listed and there’s no custody dispute, this will work.
  • Custody Order – If parents are divorced or there’s a legal custody arrangement, please bring the official court order.


2. Proof of Missouri Residency (Strongly Encouraged)
Providing proof of residency helps us confirm eligibility for financial assistance programs. You can bring any one of these:

  • A recent utility bill (electric, water, gas)
  • Lease agreement or mortgage statement
  • Official mail from a government agency (tax documents, Social Security, Medicaid)
  • Driver’s license or state-issued ID with your current address


Why is this important?
Proof of custody is required for every client to ensure we follow legal guidelines. Proof of Missouri residency isn’t required for services, but it can help us connect you with financial assistance if needed.
