Addressing the Needs of the Communities We Serve
Freeman Health System is a founding stakeholder of the Ozarks Health Commission (OHC), formed in 2015. OHC partners have a vision of using a systematic, data-driven process to inform decisions and guide efforts to improve community health and wellness on a regional level. This partnership worked throughout 2015 to develop a comprehensive community health needs assessment for the Joplin Community, which provides an up-to-date picture with which to strategically address health concerns in Freeman’s service areas. 2016 Freeman Community Health Needs Assessment and Implementation Plan.
Every three years, Freeman Health System publishes a comprehensive community health needs assessment for the Joplin community. The assessment provides an up-to-date picture with which to strategically address health concerns. The most pressing needs are prioritized, and a plan to improve health is developed. As each period comes to a close, Freeman reviews the efforts made towards these prioritized health needs.

Full printed copies of the CHNA and CHIP are available upon request via an email to CommunityHealth@freemanhealth.com or by calling 417.347.4987. Visitors to the health system may also call to request a copy for review at each of the health system information desks.
