Patient feedback is vital to our mission of improving the health of the communities we serve through contemporary, innovative, quality healthcare solutions.

Freeman has partnered with HealthStream® Research to conduct patient satisfaction surveys.

In a time where you, the patient, exercise more involvement with your medical care, it is essential for us to know if you are really satisfied with the quality of service provided by Freeman. HealthStream Research provides qualitative and quantitative feedback on the quality of care we provide to help us improve patient satisfaction.

We would like to know if you are thrilled with our services—and if you’re not, where and how we can improve. But we need your help!

What to expect

Freeman will randomly select patients from various areas within the health system to contact for feedback.

After your hospital visit you may receive a telephone call from one of our surveyors at HealthStream Research. The call will only take a few minutes of your time, and the feedback you provide will help us improve service.

We protect your privacy

Rest assured your privacy is of utmost importance to us, and your feedback will remain anonymous. Please consider HealthStream Research surveyors an extension of our team.

The choice is yours

If you receive a call from HealthStream Research, and would prefer not to provide feedback, simply tell the phone surveyor you don't want to participate in the survey.
